This presentation shows just how easy it is to fill in your CHY2 form when you are paying your contribution to the development fund.  Please note that the CHY2 form should only be filled in by those who are solely PAYE assessed.  If you are in receipt of any self-assessed income, please do not fill in the CHY2 form.
